Latch & Co. is currently in-network with Tricare, HMSA PPO, and Cigna through Wildflower Health. Patients without these plans can be seen for private pay, and most HSA/FSA accounts cover lactation services.
For Tricare families, all visits bill under CPT 99404 with no prior authorization required under the CBSD program — which means fast access for your military patients.
If you have a patient who isn't sure about coverage, encourage them to text "Benefits" to (865) 280-0783 and we'll verify for them.

How to make a referral

Download the referral form below and fax it to (865) 297-4923, or call us directly at (865) 280-0783. Both options work — whatever is easier for your workflow.

We reach out to your patient
Once we receive the referral, we'll contact your patient directly to get them scheduled. Typically within 1–2 business days. Urgent cases (NICU discharge, weight concerns) are prioritized.

Care coordination back to you
After the visit, Kelsey sends a summary back to your office if requested — keeping you in the loop on feeding progress, especially for weight-gain concerns or NICU follow-ups.
